Outpatient

Referring patients to outpatient rehabilitation services

Rehabilitation Admission Criteria

Orthopaedic Rehabilitation – Reduced mobility, function or confidence following:

  • Elective orthopaedic surgery
  • Fractures – visiting Orthopaedic Surgeon available for consultation reducing need to attend public fracture clinic

Neurological Rehabilitation

  • Stroke – improving mobility and function following stroke
  • Parkinson’s Disease optimisation including medication adjustment (visiting Neurologist available for consultation)

Reconditioning – Reduced mobility, function or confidence following acute illness, fall or injury

  • Falls without fracture – including falls education, fear of falling, reconditioning, occupational therapy review
  • Recovery following cardiorespiratory illness (exacerbation of heart failure, COVID, Influenza or bacterial pneumonia)
  • Recovery following prolonged hospital admission or deconditioning
  • Back pain – improving pain management, mobility and return to daily activities

Amputee

  • Upper limb strengthening, balance retraining, compensatory strategies, wheelchair prescription, equipment and home modifications

Subacute Medical Admissions

Infections: Non-complex infection requiring short term IV antibiotics (e.g., cellulitis, urinary tract infections, respiratory infections without significant oxygen requirement)

Wound Management: Including complex post-surgical wounds, diabetic and vascular ulcers, pressure injuries and VAC dressings – visiting Vascular Surgeon available for consultation

New Stoma management and education

Pain Management with support from visiting Pain Specialist

  • Bariatric patients >150 kg or requiring 3 assist with lifter
  • High Cost PBS Medications
  • Behavioural concerns, aggressive behaviour or unstable mental health
  • Patients at risk of absconding
  • Paediatric patients
  • Respite Care (no contract for respite or patients awaiting nursing home placement only)

Referral Flow

1) Patient meets admission criteria

Requires in‑patient admission (Rehabilitation or Medical) and is medically assessed as stable for transfer.

3) Intake review

LDH Intake Manager assesses suitability, liaises with admitting doctor, and completes fund check.

4) Approval & Bed Allocation

Health fund confirmed and bed available.

Next: Intake Manager contacts patient directly to arrange admission date and time.
